Planning Support Specialist

 

Grant funded temporary contract position. Position open until filled.  

 

Dancing Sky Area Agency on Aging is looking for a self-motivated, innovative person that enjoys working on new projects. This position is time limited ARPA grant funded through September 30,2025. Hours will fluctuate but anticipate 15+ plus hours per week.

 

Work would be looking at supporting care planning work for older adults that need care support to reduce their risk of maltreatment and support safe independent living. Strong skills in planning, organization and follow through in a high paced changing environment. Strong computer skills needed.  Travel within the 21-county planning and service area may be needed at times.

 

Specific responsibilities:

• Tracking of project and persons served

• Outreach and relationship building

• Support planning skills for individuals and families

• Strong understanding of aging services

• County coordination and support in building referrals

• Other duties as needed to complete the pilot project

Experience Required:

Preferred bachelor's degree from an accredited college or university in nursing, social work, gerontology, or related human services field. Experience in care coordination and case management OR a minimum of three years’ work experience is preferred.

 

Other requirements:

• Successful completion of criminal background check 

• Flexibility in scheduling and hours week by week
